How to Create Quizzes in Microsoft Teams Method 1 – Use Forms. The first step involves creating your quiz in Microsoft Forms. In order for this method to work, you need to use the same account to login into MS Forms and Teams. To create your quiz, add a title and a short description. Then hit the + Add new button to add the questions. Microsoft Teams, the hub for team collaboration in Microsoft 365, integrates the people, content, and tools your team needs to be more engaged and effective.

Microsoft Forms is an application which allows you to create shareable surveys, quizzes and polls in minutes.

Forms tracks participant responses, providing real-time results and analytics for your survey. Forms was originally only available on Office 365 Education, however Microsoft have since rolled out Forms to Office 365 commercial customers—which includes Chorus! So, I'm going to tell you what we think of Microsoft Forms here at Chorus and how to use it…

Which Office 365 plans include Microsoft Forms?

Forms is available to Office 365 Education users and to businesses that have any of the following commercial Office 365 plans: Office 365 Business Essentials, Office 365 Business Premium, and Office 365 Enterprise E1, E3, and E5 plans (also E4 for anyone who purchased this plan before it was retired by Microsoft).

Microsoft have also made Forms available to anyone with a personal Microsoft Account.

Recently, Microsoft released Microsoft Forms Pro. This enhanced version gives you the ability to apply company branding, integrate with Dynamics 365 and also set up workflow automations. However, in this article, we're going to focus on the standard version of Forms.

Where to find it

To find the app, open your Office 365 Home Page and look at your list of apps. If Forms isn’t showing, click ‘Explore all your apps’ and you’ll find it listed in there.

Creating a Form

Right, let’s run through the basics of creating a form. Inside the app, you’ll be presented with the Forms home page. As you can see, you can choose to create a new form or quiz.

When you select a new form, you can simply input the form details and add an image.

You can easily add questions of a variety of question types to your survey.

The ‘Choice’ option allows you to add multiple-choice questions and as many potential answers as you wish to include. You can also determine whether the respondent can provide multiple answers and whether completing the question is mandatory by toggling the ‘multiple answers’ and ‘required’ functions on/off. You can also add an image if it gives context to the question – see example below.

It’s easy to simply add more and more questions to your form as you go on.

Ms Teams Forms Hack

With the ‘text’ option, your respondents can answer your question with a free-text response - they can write whatever they like in the text box.

Date-related questions present the option to select a date from a calendar in response to a question, while ‘rating’ questions allow you to rate something using a scale of numbers or stars.

After adding your questions, you can easily change the appearance of your form by adding a ‘theme’. You can simply pick from the different colours and graphics options, before using the ‘preview’ button to see what the final survey will look like to your respondents.

Sharing your Form

Sharing your survey is so simple with Microsoft Forms. You can choose whether the survey is only shareable to people within your organisation or just anyone that gets hold of the link. You can also share survey templates and collaborate on survey design.

By copying the link, you can paste it anywhere e.g. in an email, on a webpage or social media – the possibilities are endless! Alternatively, you can generate a QR code – which requires someone to scan the code using their mobile device to access the form.

The other two options include ‘Embed’ and ‘Email’. The embed option provides some HTML code which you can paste in a webpage, while ‘email’ is exactly what it says on the tin.

Responses and Results

Once you’ve shared your new survey, you will likely want to review the responses. In your survey ‘responses’ tab, you will find a dashboard with analytics of the data submitted by your survey respondents.

As you can see, you can quickly find out how many people have submitted responses, the average time it takes a respondent to complete the form and the status of the form. Following this, you can see an overview of the answers for each of your survey’s questions. For a multiple-choice question, you will see the number of responses for each option and a pie chart.

Luckily, it seems that including a cat meme in my Chorus blog wasn’t inappropriate after all – everyone loves cats, right?

For any question type, you can see a full breakdown of all the different answers. Note how each respondent is provided with an ID. This allows you to cross-compare answers between different questions. If you share your survey with people inside your organisation, you can select the option to capture their names – otherwise the data is anonymised.

As you can see, 4/5 respondents chose Chorus as their favourite UK-based Managed IT Services Provider! I’d love to say that there wasn’t any bias here – but there was.

With ‘rating’ questions, the overview shows you the average rating from all of the responses.

Finally, you can open the data in an Excel spreadsheet in case you want to do more detailed statistical analysis – or you could even put the data into Power BI.


In summary, Forms is a very simple and intuitive application, which is a fantastic solution for businesses that have it included within their Office 365 subscription. The software creates visually attractive forms which provide a superb user experience – including on mobile devices.

Some survey software such as SurveyMonkey or Typeform have free plans, but these typically have limits on what you can do before you need to upgrade to the paid version. For example, you might only be able to add 10 questions and limited to 100 responses. More advanced features such as adding conditional questions or branching are possible with Microsoft Forms, making this an effective survey solution that is worth paying attention to. Even better, IT administrators can retain control and turn off access to users if required – allowing your organisation to keep control of the applications that staff can use.

All in all, Forms is a great option for your business – especially as it’s included with your Office 365 licensing cost.

This app template is built on the SharePoint Framework and loaded into a tab within a team. Groups can leverage this tool to help manage program membership, provide a leaderboard and event types for logging, and tools to overlay digital badges to program participants.

The Get Started app template allows you to bring the power of Microsoft 365 learning pathways inside of Microsoft Teams. This app template allows you to grant easy access to specific training pages or other intranet assets and load the content directly within Teams. You can also change the app name or logo to match your company branding.

Appointment Manager

Appointment Manager is a Teams app template to help businesses create, manage, and conduct virtual appointments with consumers through Teams. New appointment requests from consumers are visible in Teams channels, where they are quickly assigned and reassigned to staff in a team. Appointment requests are viewed at team or personal levels through custom tabs. Every appointment is associated with a Teams online meeting, hence the staff and consumers can easily join the meeting at the scheduled time.

The app template integrates with Microsoft Bookings for easy appointment management. Scheduled appointments automatically appear on assigned staff members' calendars, and consumers receive customizable email notifications and reminders with embedded meeting links.

Ask Away is a Microsoft Teams bot that enables users to conduct Question and Answer, called Q&A sessions within Teams. The Attendance app is a Power Apps tab that are pinned in a team. It is designed to record presence in settings, such as learning and training environments. Users can mark or edit attendance for up to 30 days in the past and view summarized attendance reports for an entire group or individual attendees. Book-a-room is a Microsoft Teams bot that allows users quickly to find and reserve a meeting room for 30, 60, or 90 minutes starting from the current time. The default time is 30 minutes. Building Access

Building Access is a Microsoft Power Platform based app that supports the administration of building occupancy thresholds and social distancing norms by enabling facilities directors to manage, track, and report employee on-site presence. Celebrations is a Teams app that helps team members to celebrate each others' birthdays, anniversaries, and other recurring events. It remembers special occasions of all the team members and sends a friendly message in all the teams selected at the time of event creation, to make the team members feel special on their day.

The app provides an easy interface for all the team members to personally add and view their events and also allows the user to select the teams in which the events gets shared.



Checklist is a custom Microsoft Teams messaging extension app that enables you to collaborate with your team by creating a shared checklist in a chat or channel. Classroom Drop-in

FAQ Plus

Conversational Q&A bots are an easy way to provide answers to frequently asked questions by users. But, most bots fail to engage with users in meaningful way because there is no human in the loop when the bot fails. FAQ bot is a friendly Q&A bot that brings a human in the loop when it is unable to help. One can ask the bot a question and the bot responds with an answer if it is contained in the knowledge base. Remote Support

Scrums for Group Chat is a supportive scrum assistant that enables group chat members to run asynchronous stand-up meetings and easily share their daily updates. It allows all members of the group chat to contribute to the scrum and view the updates made by others in the running scrum.

Share Now

The Share Now app promotes the positive exchange of information between colleagues by enabling your users to easily share content within the Teams environment. Users engage the app to share items of interest with team members, discover new shared content, set preferences, and bookmark favorites for later reading.

SharePoint List Search

Collaboration in Microsoft Teams quite often references information contained within items in a SharePoint list. Paste a link to the item in question forces everyone to switch context away from the conversation, find the needed information, then return to Teams to continue the conversation. As the conversation continues people have to switch back to the reference item multiple times to verify new comments and refresh their memories of the information contained within the item. This context switching creates a barrier to smooth collaboration.To resolve this problem, the List Search app template is used. Many users use SharePoint to power some of the core workflows in their organizations. However, collaborating around lists is difficult. Using the List Search app template in Microsoft Teams, users can insert information from SharePoint list items directly within a chat conversation to alleviate the context-switching caused when simply inserting a link into a chat. The information is inserted as an easy-to-read auto-formatted card, helping the users stay engaged in the conversation.

Staff Check-ins is a Power Apps based app that enables oversight communication between your business and field personnel. Time Tally

A project can include multiple tasks, and various projects can be assigned to employees. Managers are required to understand the project progress through the time spent by the employees on these tasks. This can be a cumbersome activity, as the employees need to fill in the timesheets. Time Tally app enables employees to fill their timesheets quickly, using the mobile device, and managers do not have to follow up with employees on the timesheet entry. Managers get to view the project utilization based on resources, and they can approve or reject the entries. Reminder notifications are sent to ensure timesheet compliance. Also, historical data and utilizations are available for analytics.

Virtual Rounding

Hospital and emergency room providers make many rounds per day. These quick check-ins on patients are intended to provide a status check on how the patient is doing and ensure that the patient’s concerns are addressed. While rounding is an essential practice to ensure patients are being monitored by multiple types of providers, they represent a huge drain on PPE, because for each visit, from each provider, a new mask, and new set of gloves are used.
